(1)  A declaration by a gaoler for the purposes of section 121A(3) of the Act is to be –

(a) in accordance with Form 11; and

(b) retained in the prison records.

(2)  As soon as practicable after making the declaration, the gaoler is to forward a copy to the clerk of the court which imposed the sentence to which the declaration relates.
