(1) An election, direction, claim or application under this Part (a) must be in writing signed by the person entitled to make the election, direction, claim or application; and(b) must be in the form determined by the Commission; and(c) subject to subregulation (2) , is not valid until it is received by the Commission; and(d) takes effect when it is received by the Commission, except that the Commission may, if it thinks fit, accept that an election, direction, claim or application lodged with the Ambulance Service takes effect from the day it is so lodged.(2) If the Commission may, subject to the terms and conditions, if any, that it imposes, accept the election, direction, claim or application and deal with it as if it had been received within the time so prescribed.(a) an election, direction, claim or application under this Part is not received by the Commission within the time prescribed for the election, direction, claim or application; and(b) the Commission is satisfied that, having regard to all the circumstances of the case, it is desirable that the election, direction, claim or application should be accepted
